Sinclair Lewis acknowledges Dr. Paul H. DeKruif for his extensive contributions to the bacteriological and medical material in the book, as well as his help in planning the fable, his realization of the characters as living people, and his philosophy as a scientist. Lewis also records their months of companionship while working on the book in various locations around the world.
Chapter 2
Arrowsmith
The opening chapter introduces Martin Arrowsmith as a young boy in the small town of Elk Mills, revealing his early curiosity about medicine and science through his apprenticeship with Doc Vickerson. It establishes his restless, ambitious nature and his initial steps toward a medical career.
Chapter 3

Martin Arrowsmith is offered the position of Assistant Director at the McGurk Institute by Rippleton Holabird, with the prospect of becoming Director. Joyce Lanyon urges him to accept, but Martin recoils from the administrative and social demands of the role, seeing it as a betrayal of his scientific ideals. After a heated argument with Joyce, he decides to leave the Institute and join Terry Wickett in Vermont to pursue pure research, abandoning his comfortable life and family.
